A few more questions. The answers may or may not help me help you, but
bear with me.

What type of server are the HBAs in? Also an IBM? OR perhaps a Dell?
Or homebuilt or other brand? Also what model? And what model/type etc.
is the SAN?

Also, I'm not sure about the IBM ones, but quite a few SANs come with
their own proprietary multipathing software/drivers. You might want to
check that's not a requirement for yours to work.

What's the output of
$ cat /etc/multipath.conf |egrep -v "(^#|^$)"

Are you SURE of the above? What's the outputs of:
$ fdisk -l
$ lsmod

and of

> $ dmesg | grep sda
> [  114.435441] sd 0:0:0:0: [sda] 524288000 512-byte hardware sectors (268435 MB)

$ dmesg |egrep "sd(a|b|c|d)"

> $ lspci | grep QLogic

Fianlly, what distro and output of uname -a
